The collapse was caused by the failure of connections supporting the fourth-floor walkway ceiling rods. That sent the fourth-floor walkway crashing onto the second-floor walkway directly beneath, and both fell to the floor. According to a subsequent investigation, the connection failure resulted from a design change during construction that doubled the load on the connections, far exceeding their intended capacity.
